Urgent Medical Device Recall Notice – December 2009

and concerns the following product:

neXus 1 Model #4520 manufactured prior to October 30, 2008 and with serial numbers between 10000-35 and 16395-35
or the following individual serial numbers:
16791-35, 17627-35, 19415-35

neXus 2 Model #4515 manufactured prior to October 16, 2008 and with the serial numbers between 10000-45 and 18015-45

neXus 3 Model #4500 manufactured prior to September 18, 2008 and with the serial numbers between 10000-40 and 70626-40
or the following individual serial numbers:
70761-40, 71130-40, 71136-40, 71149-40, 71150-40, 71158-40, 71160-40, 71162-40, 71196-40

Please note that it has come to our attention that neXus rollators manufactured prior to the dates indicated above may experience a weakening just below the weld on the rear frame which could result in breakage when used by consumers that are 187 lbs (85 kg) or more.   If breakage occurs, the user could fall and suffer injury including broken bones and bruising.  We have had 75 reported incidents of the rear frame breaking on the neXus 1 and 2 models of which six (6) have resulted in falls and bruising to the user.  We have had 145 reported incidents of the rear frame breaking on the neXus 3 model.  No injuries have resulted from the incidents involving the neXus 3 model.  Please note that only serial number(s) indicated above are affected by this issue.

In order to correct this issue, we have designed and manufactured a steel support bracket that can be assembled to the affected neXus frames.  To determine if a particular neXus frame is affected and a support bracket is required, please refer to the plastic cap on the frame into which the push handle is inserted.  Affected neXus frames are those with a 1.2mm rear leg - these rollators will have a plastic cap with a "U-shaped" cut out (see diagram below).

Modification is to be immediately carried out by an authorized dealer only.

FRONT FORK BEARING
Medical Device Recall Notice – December 2009

This recall concerns the following product:

neXus 1 Model #4520 with serial numbers between 14773-35 and 16062-35
neXus 2 Model #4515 with serial numbers between 16213-45 and 17708-45
neXus 3 Model #4500 with serial numbers between 66982-40 and 72867-40

We have identified an issue in which the front fork bearing may loosen causing the front wheel to fall off the neXus frame when the rollator is raised off the ground.  If the front wheel falls off, the user could fall and suffer injury incuding broken bones.  We have had nine (9) reported incidents in which the front wheel has fallen off the frame.  No injuries have resulted from these incidents.  Please note that only the serial number(s) indicated above are affected by this issue.

Modification is to be immediately carried out by an authorized dealer only. 


FRONT WHEEL FORK
Medical Device Recall Notice – December 2009

This recall concerns the following product:

neXus 1 Model #4520 with serial numbers between 16668-35 and 18640-35
neXus 2 Model #4515 with serial numbers between 19008-45 and 20735-45
neXus 3 Model #4500 with serial numbers between 76025-40 and 92072-40

We have identified an issue with these products in which the front wheel forks may crack with usage which may ultimately lead to breakage.  If the front wheel fork cracks and breaks, the user could fall and suffer injury incuding broken bones.  We have had twenty-one (21) reported incidents of the front wheel fork cracking [or breaking].  No injuries have resulted from these incidents.  Please note that only the products with serial number(s) indicated above are affected by this issue.

Modification is to be immediately carried out by an authorized dealer only. 


LOWER BRAKE ROD
Medical Device Recall Notice – December 2009

This recall concerns the following product:

neXus 3 Model #4500T (Tall Handles Only) with serial numbers between 91164-40 and 102792-40

We have identified an issue with these products in which the brake could potentially jam in the locked position and not release resulting in the user being unable to move the rollator.  There is likely still some risk to users, for example, if the brake were to jam up when the user was away from home and they rely on the walker to get around.  There have been five (5) reported incidents of the brake jamming in the locked position during use.  No injuries have resulted from these incidents.  Please note that only the serial number(s) indicated above are affected by this issue.

Modification is to be immediately carried out by an authorized dealer only.
